Where this comes from

The official form is published by tac.vic.gov.au; this page is a completion guide. Last checked on 2026-10-23.

When to use it

Use when a serious injury certificate application to the TAC will be lodged more than six years after the injury, or more than six years after a minor turns 18. Do not use it if the lawyer knows the TAC does not indemnify the defendant.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

The tool may diary the six-year date from the injury date or the eighteenth birthday the lawyer confirms, and list the defendants already named on the file; the lawyer completes and sends the TAC form.

AI may

  • Diary six years from the injury date the lawyer confirms, or six years from the date a minor turns 18
  • List proposed defendants and vehicle registrations already recorded on the file
  • Check that the anticipated lodgement date the lawyer chooses is within six months of the notification date

AI must not

  • Must not decide that the TAC indemnifies a defendant
  • Must not choose the anticipated lodgement date or write the other comments
  • Must not describe the accident or the injuries in the form
  • Must not submit the form

The lawyer checks

  • The serious injury application will be outside the six-year period the form states
  • The lawyer does not know of a defendant the TAC does not indemnify. If the lawyer does know that, this form is not used
  • Name, date of birth, accident date and claim number match the TAC record
  • The anticipated lodgement date, including Chapter 5 material, is within six months of notification
  • Health information stays in a firm-controlled tool. Check ASCR r 9, APP 11, and whether any disclosure would be a notifiable breach under the Privacy Act 1988 (Cth).
