Where this comes from

The official form is published by worksafe.qld.gov.au; this page is a completion guide. Last checked on 2026-10-23.

When to use it

Use when a claimant asks the Workers' Compensation Regulator to review an insurer's decision, or a failure to decide, under section 542. QIRC Forms 9, 9A, 9B and 9C are the later appeal, if an appeal is brought. Form 542P is the premium and policy review and is not this form.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

AI may check the review form against the decision on the file and list missing grounds documents; it writes nothing in the grounds or the signature, and the lawyer lodges it.

AI may

  • Check the draft is the current file from the official page and list every blank field and missing attachment
  • Index the documents already on the file that the form calls for, with date and author
  • Turn the published giving and response steps into a checklist for this matter

AI must not

  • Must not write the grounds of review or any statement the applicant signs
  • Must not decide whether the application is in time or whether special circumstances exist
  • Must not lodge the review or start the online form

The lawyer checks

  • The decision is reviewable under chapter 13 and Form 542.3 is the approved form
  • Section 542(1) or (4) is applied to the notice and the reasons, not only to one of the form's two sentences
  • The grounds are the applicant's
  • An appeal, if any, uses the industrial commission or an industrial magistrate as section 546 states, and not this form

Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
