Where this comes from

The official form is published by niis.qld.gov.au; this page is a completion guide. Last checked on 2026-10-23.

When to use it

Use when screening a Queensland motor-accident injury for interim participation in the National Injury Insurance Scheme. The scheme is no-fault. The insurer's application form is a different document. The lawyer does not treat the form's injury list as a substitute for the Act and regulation, which were not opened.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

AI may check the interim application against the file and list missing assessments; it writes nothing in the declaration or the medical certificate, and the specialist completes the clinical part.

AI may

  • Check the draft is the current file from the official page and list every blank field and missing attachment
  • Index the documents already on the file that the form calls for, with date and author
  • Turn the published giving and response steps into a checklist for this matter

AI must not

  • Must not write the declaration, the medical certificate, or any FIM or WeeFIM score
  • Must not decide eligibility or whether an injury meets the statutory definition
  • Must not sign or lodge the application

The lawyer checks

  • The accident was in Queensland on or after 1 July 2016
  • The current interim-participant form is used, not the insurer form
  • A qualified medical specialist completes the medical certificate, and a credentialed assessor completes any FIM or WeeFIM
  • The person signing for the injured person is 18 or older

Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
