Where this comes from

The official form is published by mcv.vic.gov.au; this page is a completion guide. Last checked on 2026-10-23.

When to use it

Use when parties in the Magistrates' Court are asking the Court to refer medical questions to a Medical Panel. The parties or their practitioners complete the Court form. Check whether Medical Panels also requires its Court Referral Form 2026.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

The tool may index the reports proposed for a referral; the practitioners complete the Court form and settle the medical questions, and the tool writes nothing that is filed.

AI may

  • Index medical reports and clinical notes already on the file that the lawyer is considering for a referral bundle
  • Note whether the proceeding is in the Magistrates' Court, the County Court, or another body the Medical Panels page names as a referrer

AI must not

  • Must not write the medical questions or any other text that is filed
  • Must not describe the 2018 form's questions. The PDF and the Word file were not opened
  • Must not treat the 2018 form as repealed. The Court page still publishes it

The lawyer checks

  • The Court page says the form should be completed by parties or their practitioners and submitted with all applications for referral to a medical panel. It is dated 1 January 2018 and was last updated on 13 November 2018
  • Medical Panels says a court referral should use its Court Referral Form 2026. The file name on that page is Court-referral-form-2025-07.pdf. Check which form the Court and Medical Panels currently require
  • Medical Panels says workers compensation referrals are made by WorkSafe and its agents, Workplace Injury Commission officers, self-insurers, the Magistrates' Court, the County Court, and the body the page calls the Victorian Civil Appeals Tribunal (VCAT)
  • Email to Medical Panels, when the page's path is used, is referrals@medicalpanels.vic.gov.au
  • Health information stays in a firm-controlled tool. Check ASCR r 9, APP 11, and whether any disclosure would be a notifiable breach under the Privacy Act 1988 (Cth).

Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
