Work capacity certificate for a Queensland workers' compensation claim

Link to the WorkSafe work capacity certificate page: the claim is not valid without it. Only a doctor, a nurse practitioner for a minor injury, or a dentist for an oral injury may issue it.

Where this comes from

The official form is published by worksafe.qld.gov.au; this page is a completion guide. Last checked on 2026-10-23.

When to use it

Use when a Queensland workers' compensation application needs the certificate in section 132. The practitioner issues it. Allied health cannot. The sample on the page is an image, and some doctors use an older version.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

AI may check that a certificate is on the file and list the injuries it names; it writes nothing in the diagnosis, capacity or certificate, and the practitioner completes it.

AI may

  • Check the file for a certificate that names every injury in the application
  • Record the practitioner's name, the date, and which injuries are listed
  • Flag a certificate that appears to be from an allied health provider

AI must not

  • Must not write the diagnosis, the history, the capacity, the restrictions, or any other clinical content
  • Must not issue, sign or alter a work capacity certificate
  • Must not decide whether the injury is minor or oral, or which practitioner may sign

The lawyer checks

  • The certificate is the approved form under section 132 and covers every injury to be assessed
  • The signatory is a doctor who attended, a nurse practitioner for a minor injury, or a dentist for an oral injury
  • An older version, if the doctor used one, is accepted only if the insurer accepts it
  • The medical-provider instructions page was not opened

Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
