Client authority and records request pack for a personal injury claim
Signed client authority plus a schedule of record requests for a personal injury claim (treating doctors, hospitals, ambulance, employer and payroll, Medicare and Centrelink, insurers), with a tracking table so gaps in the record set are visible early.

When to use it
Use at the start of a personal injury matter, after the retainer is signed, to collect the records needed for the chronology, liability and damages. Pair it with the medical chronology and record-gap tasks. Use the agency's own form where it requires one (for example Medicare claims history or a notice of past benefits).
Used for these tasks
- Find gaps and duplicates in a medical-record set
- Organise chaotic PI evidence into a fact base
- Build a medical chronology from the record set
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
AI may fill the authority and the request letters from the intake inside the firm's approved template and keep the tracking table current; staff check the authority and each recipient before anything is sent.
AI may
- Fill the client details and the list of providers from the intake form and Medicare claims history
- Generate one request letter per provider from the approved template, quoting the authority
- Update the tracking table as records arrive and flag providers, periods or record types still missing
AI must not
- Must not add providers or record types the client has not agreed to
- Must not send requests without the signed authority attached
- Must not process medical records in a tool outside the firm's approved environment
The lawyer checks
- The client understands what will be requested, from whom and why, and has signed the authority
- Requests are limited to records relevant to the claim, including the pre-injury period the claim needs
- Agency-specific forms are used where required, and sensitive records (for example mental health or sexual assault counselling records) are handled under the firm's protocol
Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
