Where this comes from

The official form is published by ucprforms.nsw.gov.au; this page is a completion guide. Last checked on 2026-10-23.

When to use it

Use when particulars are filed in a New South Wales compensation to relatives proceeding. Personal injury proceedings that are not brought for dependants use Form 14.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

The tool writes nothing in the filed particulars, and it may only list the people and dependency records already on the file for the lawyer who writes the form.

AI may

  • List each person for whom the proceeding is brought, with the relationship recorded on the file
  • Index wage, tax and household records that bear on dependency
  • List documents already served and documents still to be served

AI must not

  • Must not write the summary of the claim or the details of dependency
  • Must not describe the family's circumstances in words that are not in a document the lawyer has accepted
  • Must not sign or file the form

The lawyer checks

  • The form is Form 15 version 2. It cites UCPR 15.13(2)(b) and 15.13(2)(c)
  • For each person: name, address, relationship to the deceased, and marital status
  • Documents served in support of the claim, and other documents served, are listed, with reasons where a document cannot be served with the statement
  • The legal representative or the party signs
  • This guide is red because the particulars are filed. The form is not an affidavit
  • Health, wage and other personal information stays in a firm-controlled tool. Check confidentiality under ASCR r 9 and APP 11, and whether any breach is notifiable under the Privacy Act 1988 (Cth).

Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
