Where this comes from

The official form is published by icare.nsw.gov.au; this page is a completion guide. Last checked on 2026-10-23.

When to use it

Use when a person who was severely injured in a motor accident in NSW applies to Lifetime Care, usually while in hospital after a Severe Injury Advice form. A Lifetime Care application is not a CTP personal injury claim.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

The tool may list the accident, hospital and insurer documents already on the file, and it writes nothing in the signed application, the consent or the medical certificate.

AI may

  • List whether a signed application, a consent, a police report and a specialist medical certificate are on the file
  • Record which assessment tool the page names for the injury type on the file
  • Note the interim two-year period once an acceptance date is known

AI must not

  • Must not write the signed application, the consent, or the specialist's medical certificate
  • Must not decide eligibility or whether an injury meets a guideline
  • Must not describe the word-document fields. Those documents were not opened
  • Must not submit the application

The lawyer checks

  • The application includes a signed form and a consent for icare to contact doctors, police, ambulance and insurers. Include a police report if the applicant has one. A medical specialist completes the medical certificate
  • Assessment tools named on the page: FIM and WeeFIM for brain injuries and burns; the ASIA Scale for spinal cord injuries; amputations by percentage loss or equivalent impairment, with no extra tool; permanent blindness means legally blind in both eyes, with no extra tool
  • If eligible, the person is an interim participant for two years. icare pays reasonable and necessary treatment, rehabilitation and care from the motor accident injury. Near the end of the two years, a lifetime participation application is made
  • The person or a CTP insurer can dispute an eligibility decision
  • The page says a Lifetime Care application is not a CTP personal injury claim. CTP Assist is 1300 656 919 and ctpassist@SIRA.nsw.gov.au
  • If the person has left hospital, the page gives 1300 738 586. The interim application docx and the Severe Injury Advice docx were not opened
  • Health, wage and other personal information stays in a firm-controlled tool. Check confidentiality under ASCR r 9 and APP 11, and whether any breach is notifiable under the Privacy Act 1988 (Cth).

Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
