Where this comes from

The official form is published by worksafe.qld.gov.au; this page is a completion guide. Last checked on 2026-10-23.

When to use it

Use when a worker applies for Queensland workers' compensation for an injury. The application is lodged with WorkCover Queensland or the employer's self-insurer. It is not the notice of claim for damages, and an employer's eight-day injury report is not the claim.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

AI may check the lodging page against the file and list the certificate and application that are missing; it writes nothing the worker signs, and the lawyer checks section 131.

AI may

  • Check the draft is the current file from the official page and list every blank field and missing attachment
  • Index the documents already on the file that the form calls for, with date and author
  • Turn the published giving and response steps into a checklist for this matter

AI must not

  • Must not write the application, the worker's answers, or any declaration on the online form
  • Must not complete or alter the work capacity certificate
  • Must not decide when the entitlement to compensation arose, or whether a waiver applies
  • Must not sign or lodge the application

The lawyer checks

  • The application is in the approved form and is lodged with the insurer, as section 132 requires
  • A work capacity certificate accompanies it, from a doctor, a nurse practitioner for a minor injury, or a dentist for an oral injury
  • Section 131 is applied from the date the entitlement arose, which is not assumed to be the first consultation
  • The employer is identified as WorkCover-insured or self-insured before the lodging path is chosen

Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
