Where this comes from

The official form is published by tac.vic.gov.au; this page is a completion guide. Last checked on 2026-10-23.

When to use it

Use when a family is claiming funeral expenses or dependency benefits from the TAC after a person has died in a transport accident. The claimant completes the official form. For an injured person's own benefits, use the online claim lodgement form.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

The tool may index the death certificate, funeral invoice and relationship documents the lawyer has collected; the family completes the official form and the lawyer checks every statement about the death and the dependency.

AI may

  • Index documents already held: death certificate, funeral invoice, and proof of relationship or dependency
  • List which of funeral expenses, dependency benefits, or both, the instructions say are being claimed
  • Check the draft index against the TAC page, which offers an online form and a PDF

AI must not

  • Must not describe how the person died or the family's loss
  • Must not decide who is a dependant
  • Must not write answers into the online form or the PDF
  • Must not put health or family information into a tool that is not approved for it

The lawyer checks

  • The death was in a transport accident and the correct TAC form is used, not the injured-person lodgement form
  • Online form or the current PDF, not a stored copy
  • Funeral invoice and dependency documents match the people named
  • Health information stays in a firm-controlled tool. Check ASCR r 9, APP 11, and whether any disclosure would be a notifiable breach under the Privacy Act 1988 (Cth).

Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
