Where this comes from

The official form is published by tac.vic.gov.au; this page is a completion guide. Last checked on 2026-10-23.

When to use it

Use when a person injured in a Victorian transport accident is ready to lodge a no-fault claim with the TAC. The claimant completes the official form. For a death, use the funeral and dependency benefits form.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

The tool may list the documents the TAC page asks for and diary the 12-month date from the accident date the lawyer confirms; the claimant completes the official form and the lawyer checks every answer.

AI may

  • List the accident, injury, police and income items the TAC lodgement page says to gather, against what is already on the file
  • Diary 12 months from the accident date the lawyer has confirmed, and the 10-day life of a saved draft
  • Index treating letters, discharge summaries and certificates of capacity already held, without describing the injuries

AI must not

  • Must not write the claimant's answers into the online form or describe the accident or the injuries
  • Must not decide that reasonable grounds for a late claim exist
  • Must not put medical records into a tool that is not approved for health information
  • Must not submit the form

The lawyer checks

  • Accident date taken from a document, and the 12-month date diarised
  • Police report number, or the public-transport incident number and the date it was reported
  • Written injury details from a treating practitioner or hospital, not photographs
  • Email confirmation of submission saved to the file
  • Health information stays in a firm-controlled tool. Check ASCR r 9, APP 11, and whether any disclosure would be a notifiable breach under the Privacy Act 1988 (Cth).

Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
