Where this comes from
The official form is published by service.nsw.gov.au; this page is a completion guide. Last checked on 2026-10-17.
When to use it
Use at the first contact with a person injured in a motor vehicle accident in NSW or Victoria. The claimant completes and certifies the official claim; the lawyer checks deadlines and evidence. For other states, use the local CTP scheme.
How AI may be used
Lawyer-led. The claimant certifies the claim, so AI writes nothing into the claim form and does not choose the scheme or form version. AI may calendar deadlines and index attachments for the lawyer to check.
AI may
- Calculate the 28-day, 3-month and 3-year dates from the accident date and put them in the matter diary, showing the working.
- Index the attachments (police event number, certificate of capacity, earnings evidence).
- Draft a plain-English checklist for the client of documents to gather.
AI must not
- Must not write answers into the claim form or describe the accident or injuries for the claimant.
- Must not decide which scheme applies (CTP, workers compensation, or both) or which form version to use.
- Must not be given the client's medical records in a tool not approved for health information.
The lawyer checks
- The accident date and all deadlines are confirmed from documents.
- The claim was lodged through the official channel and receipt is recorded.
- Any overlap with workers compensation has been identified.
- The client knows the legal costs rules for statutory benefits claims.
Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
