When to use it

Use when wage, leave or incident records are needed from an employer, a labour-hire agency or a host. Use a separate authority for each employer. Use the agency's own form if it will not accept this one. Do not use it to collect medical records.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

The tool may fill the employer, the period and the record list from the intake, and the lawyer checks the signed authority before it is sent.

AI may

  • Fill the employer's name and the date range from the intake questionnaire
  • Prepare one covering letter for each employer the client has named
  • List record types the client has ticked, and no others

AI must not

  • Must not add an employer or a date range the client has not agreed
  • Must not describe this authority as a statutory declaration
  • Must not send it before the client has signed it

The lawyer checks

  • The client understands which employer will be contacted and for which years
  • A host employer and a labour-hire agency are separated if both hold records
  • Wage information is kept in the firm's approved system (ASCR r 9; APP 11). Assess a loss under the Notifiable Data Breaches scheme if serious harm is likely
