Where this comes from

The official form is published by qld.gov.au; this page is a completion guide. Last checked on 2026-10-23.

When to use it

Use when the person paying the funeral costs asks Victim Assist Queensland, and the page's exclusions do not apply. A motor-accident death uses the MAIC fatal notice for those expenses. The main Victim Assist application already has its own precedent and is not copied here.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

AI may check the funeral-expenses page against the file and list the invoices that are missing; it writes nothing in the application or the consent, and the applicant submits it.

AI may

  • Check whether the page's online path or email path is the one the applicant is using
  • Index funeral invoices and receipts already on the file, and any other payment the applicant has disclosed
  • List the cost categories the page allows and the categories it excludes

AI must not

  • Must not write the application, the privacy acknowledgement, or the consent
  • Must not complete the Funeral Only PDF, which was not opened
  • Must not decide eligibility, or that a death caused by an accident can be claimed here
  • Must not lodge or email the request

The lawyer checks

  • The death was not caused by an accident, suicide, or the other excluded events the page lists
  • The applicant is a person the page says can request funeral expenses, and the costs are in the allowed categories
  • A related victim claiming online has Part A and then Part B
  • The $15,000 figure and the 35-day assessment period are read on the current page before they are repeated

Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
