Where this comes from

The official form is published by tac.vic.gov.au; this page is a completion guide. Last checked on 2026-10-23.

When to use it

Use after the TAC has granted a serious injury certificate by the proactive or fast track path under the Common Law Protocol 2026, and before a common law conference is arranged. The lawyer completes the official form.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

The tool may index documents already sent to the TAC and documents still to upload; the lawyer completes the form, the particulars of loss, and the tax-file-number declaration.

AI may

  • Index documents already sent to the TAC and documents not yet sent, by category: accident and liability, medical, financial, and other
  • Record the certificate date and whether the file says the grant was proactive or fast track
  • Flag whether the instructions claim pecuniary loss above $200,000, so the lawyer can prepare particulars

AI must not

  • Must not write the particulars of pecuniary loss
  • Must not draft a witness statement, a lay affidavit, or any other document that is uploaded
  • Must not tick or write the declaration that tax file numbers have been redacted
  • Must not decide that a certificate has been granted

The lawyer checks

  • A proactive or fast track serious injury grant is on the file, with the date and the path
  • The form is sent before a common law conference is arranged
  • Name, date of birth, accident date and claim number match the TAC record
  • The schedule includes documents already sent. Only documents not already sent are uploaded
  • Tax file numbers are redacted under clause 5.12 of the Protocols Framework Agreement, and the declaration is the lawyer's
  • If an administrator has been appointed, the supporting material is uploaded
  • Health information stays in a firm-controlled tool. Check ASCR r 9, APP 11, and whether any disclosure would be a notifiable breach under the Privacy Act 1988 (Cth).
