Where this comes from

The official form is published by tac.vic.gov.au; this page is a completion guide. Last checked on 2026-10-23.

When to use it

Use when a lawyer is asking the TAC to assess a client's impairment under the 2026 Impairment Assessment Protocols. The lawyer completes the official form. For a serious injury certificate, use the serious injury application form.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

The tool may index reports already on the file and list the injuries named in them; the lawyer completes the TAC form and settles every whole person impairment figure.

AI may

  • Index treating, medico-legal and joint medical examination reports already held, with author and date
  • List injuries named in those reports so the lawyer can match each one to a TAC claim number
  • Note whether the file shows the client is under 18, has another TAC claim, or has an administrator

AI must not

  • Must not nominate a whole person impairment percentage or an AMA Guides table
  • Must not decide stability, apportionment, causation or whether an impairment is interim
  • Must not write the injury descriptions that go into the form
  • Must not upload an EFT authority or any document

The lawyer checks

  • Name, date of birth and claim number match the TAC record exactly
  • Each injury is specific and tied to the correct claim number
  • The schedule lists documents already sent and documents still to upload
  • The suggested whole person impairment figures are the lawyer's, with the AMA Guides reference
  • The form is without prejudice and is not later annexed unless the other side agrees
  • Health information stays in a firm-controlled tool. Check ASCR r 9, APP 11, and whether any disclosure would be a notifiable breach under the Privacy Act 1988 (Cth).

Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
