Where this comes from

The official form is published by nsw.gov.au; this page is a completion guide. Last checked on 2026-10-23.

When to use it

Use when a primary victim, a secondary victim, or a parent, step-parent or guardian of a child primary victim applies for support from Victims Services NSW. Family members of a person who died from homicide or road crime use the separate pages that site directs them to.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

The tool may list eligibility dates, expenses and records already on the file, and it writes nothing in the declarations and nothing in the victim's account of what happened.

AI may

  • Record the incident date, the applicant's age at that date, and which support type is being asked for
  • Index police, medical and counselling records, invoices and earnings documents by date
  • Build a list of out-of-pocket amounts from invoices already on the file

AI must not

  • Must not write the declarations, including a declaration signed by a parent, step-parent or guardian
  • Must not write the victim's account of the incident or of its effect
  • Must not decide which recognition category or amount applies
  • Must not submit the application

The lawyer checks

  • Counselling has no time limit for a primary victim, a secondary victim, or a parent, step-parent or guardian of a primary victim who was under 18
  • Immediate-needs financial assistance is for primary victims, within 2 years, or within 2 years of turning 18 if the victim was a child
  • Economic-loss financial assistance has the same 2-year rule for primary victims and for parents, step-parents and guardians of a child primary victim, and the page states no time limit for out-of-pocket and justice-related expenses where the victim of sexual assault was a child
  • A recognition payment is for primary victims: 2 years, or 10 years for domestic violence, sexual assault or child abuse, or 10 years after turning 18 if the victim was a child, and no time limit for sexual assault of a person who was under 18
  • The page says to read the eligibility pages for detail. Those pages were not opened. If the applicant is under 18, the parent, step-parent or guardian's details are required and that person signs the declarations
  • All applicants need current government-issued identification. If the incident was reported to NSW Police, a police report need not be sent. The application can record the report
  • Health, wage and other personal information stays in a firm-controlled tool. Check confidentiality under ASCR r 9 and APP 11, and whether any breach is notifiable under the Privacy Act 1988 (Cth).
