Where this comes from
The official form is published by victimsofcrime.vic.gov.au; this page is a completion guide. Last checked on 2026-10-23.
When to use it
Use this when a child or young person who is Aboriginal or Torres Strait Islander is applying for financial assistance and the family chooses the FAS cultural support plan. A current plan may be uploaded instead if it has professional contacts and healing-journey goals.
Used for these tasks
- Record who may help a child apply for victims assistance, and where the money is held
- Prepare the application to a state victims support scheme
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
The tool indexes the plan headings and does not write the child's account or sign the parent or guardian consent.
AI may
- Record who the file names as parent or guardian, and whether a protection order is already on the file.
- List the blank headings so the lawyer can see what the template still asks for.
AI must not
- Do not sign the parent or guardian consent, and do not write that consent.
- Do not write the child's connection-to-community or healing-journey account.
- Do not invent clan, language, mob, community, or protection-order details.
The lawyer checks
- The consent asks whether a parent or guardian agrees to the FAS using the information for the application and for referrals to services or programs.
- The consent has a field for the type of protection order if applicable. Confirm the order from the file before anything is written there.
- An existing plan may be used instead. The template says it must have professional contacts and goals that relate to the healing journey.
- Health information stays in a firm-controlled tool. Check ASCR r 9, APP 11, and whether any disclosure would be a notifiable breach under the Privacy Act 1988 (Cth).
Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
