Where this comes from
The official form is published by nsw.gov.au; this page is a completion guide. Last checked on 2026-10-23.
When to use it
Use when assembling a NSW recognition payment for a primary victim of violent crime or modern slavery, or for a family victim of homicide. Lodge through the current Application for support. This page is the recognition payment guide, not that application form.
Used for these tasks
- Map a family claim after a homicide onto the published assistance amounts
- Record who may help a child apply for victims assistance, and where the money is held
- Place the published family figure next to the relationship label already marked
- Line injury reports up against recognition-payment categories before lodgement
- Check a victims' scheme decision against the categories and limits
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
The tool may set the published amounts beside relationships and dates the lawyer has already marked, and it writes nothing in the application or the person's account of the violence.
AI may
- List each family applicant beside the relationship a document already records, and beside the amount line the page prints for that description
- Enter the incident date, the date of birth and the time-limit sentence the page prints for that description, for the lawyer to check
- List the primary-victim documents the page asks for and mark which are already on the file
AI must not
- Must not choose the recognition category, decide eligibility, or say that the published amount will be paid
- Must not write the person's account of the violence or of how it affected them
- Must not sign the application or send identity documents
- Must not describe the family-victim document list. That list is on a homicide page that was not opened
The lawyer checks
- The amounts and time limits are read from the current recognition payment page. The page was opened on 23 October 2026. The Act was not opened
- The $7,500 amount line and the who-can-apply list are kept separate. Only the who-can-apply list includes the two years of living with the homicide victim
- The page points to section 25 and section 44, link text Victims Support Act 2013. Those sections were not opened. Its section 44 examples are behaviour of the deceased that contributed to the death, including past criminal activity, and money from another source, including insurance
- A primary victim's medical, dental or counselling report can be sent up to 12 months after lodgment. The page says the recognition application is closed if the report is not sent in that time
- A recognition payment approved while the applicant is under 18 is paid to NSW Trustee and Guardian to hold until the applicant turns 18, on this page. How the Trustee manages the money was not read
- Identity documents are copies. The page says not to send the original. A family member or friend who is the authorised representative also provides identification
- The application is lodged through the making-an-application page, which has its own guide. This entry does not replace that guide
- Health and other personal information stays in a firm-controlled tool. Check confidentiality under ASCR r 9 and APP 11, and whether any breach is notifiable under the Privacy Act 1988 (Cth).
Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
