Where this comes from
The official form is published by publications.qld.gov.au; this page is a completion guide. Last checked on 2026-10-23.
When to use it
Use when acting for a parent, approved carer or parenting-order parent who was injured by learning of violence against their child in Queensland and applies to Victim Assist Queensland.
Used for these tasks
- Record who may help a child apply for victims assistance, and where the money is held
- Prepare the application to a state victims support scheme
- Apply for more time to lodge a victims assistance claim
- Index the police, medical and counselling records for a victims' application
- Work out whether another payment must be notified or assistance repaid
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
The tool may index the child's records and the parent's expenses for the lawyer to check, and it writes none of the parent's own account or declaration.
AI may
- Index the documents that show the relationship to the child and the injury to the parent
- List invoices and other payments for the lawyer to check against the declaration questions
- Note the form's 3-year question and whether the child has a separate primary application
AI must not
- Must not write the account of what happened to the child or how it affected the parent
- Must not write the declaration of other payments or any signature
- Must not decide whether the person is a parent under the form, or whether an extension should be asked for
- Must not write the confirmation that all information in the form is, to the best of the person's knowledge, true and not misleading
The lawyer checks
- Form 8 version 3, effective 14 September 2023, returned in full
- The child was under 18, the violence was in Queensland, and the parent was injured by learning of it
- The person fits a limb of the form's parent definition and is not in the form's excluded group
- The child's recovery is on the child's own primary form
- Other payments declared, and section 94 followed where the applicant is a child or has a financial administrator
Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
