Where this comes from

The official form is published by publications.qld.gov.au; this page is a completion guide. Last checked on 2026-10-23.

When to use it

Use when a Victim Assist Queensland application needs the treating practitioner's certificate of the patient's injuries, or when another medical document is being used instead.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

The tool may list which part of the certificate the file appears to match, and it writes nothing in the injury, diagnosis or management sections.

AI may

  • List the treating practitioners and the dates of the records already held
  • Note whether the file describes one act, a series, or a historic act, for the lawyer to choose Part A, B or C

AI must not

  • Must not write the injury, the stated cause, a diagnosis, a diagnostic plan or a management plan
  • Must not sign or complete the practitioner's details
  • Must not decide the balance of probabilities or eligibility

The lawyer checks

  • The practitioner who treated the patient completes and signs it
  • Patient details match the application
  • The patient's consent is in place before Victim Assist is invited to contact the practitioner
  • Another medical document may be used, because the publications page says the certificate is optional

Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
