Where this comes from
The official form is published by victimsofcrime.vic.gov.au; this page is a completion guide. Last checked on 2026-10-23.
When to use it
Use this after an assistance application, or an application to vary assistance or a transitioned Victims of Crime Assistance Tribunal award, has already been submitted, and the cost is a disbursement of making that application. Do not use it for the lawyer's fees for legal services.
Used for these tasks
- Build the expenses and income loss schedule for a financial assistance claim
- Index the police, medical and counselling records for a victims' application
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
The tool indexes the request fields and does not write the declaration, the privacy acknowledgement, or any attached statutory declaration.
AI may
- List the expense lines the file already holds, with the payee, the amount and the document that evidences each one.
- Record that a lawyer uses the separate legal-costs application for fees for legal services, and this form for other disbursements.
- Diary the FAS helpline 1800 161 136 and the Marra Yattakunar team 1800 849 778 as printed on the form.
AI must not
- Do not write or sign Part D, including the declaration that the information is true and not misleading, or the collection-notice acknowledgement.
- Do not draft a statutory declaration to evidence a non-professional expense. The form allows one as evidence. The tool writes none.
- Do not use this form for a lawyer's fees for legal services.
- Do not state a cap, a time limit, or a repayment figure. None is printed on this form.
The lawyer checks
- Confirm an assistance application, or a variation application, has already been submitted. The form says the request is not used before that.
- Part D acknowledges offences in sections 65 and 66 of the Victims of Crime (Financial Assistance Scheme) Act 2022 (Vic). The lawyer reads those sections before the client signs.
- A professional expense needs an invoice or receipt with an itemisation, an ABN, the business name and contact, and the business payment details. A non-professional expense needs an invoice, a receipt, or a statutory declaration.
- Send copies. The form says original documents are not returned.
- Health information stays in a firm-controlled tool. Check ASCR r 9, APP 11, and whether any disclosure would be a notifiable breach under the Privacy Act 1988 (Cth).
Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
