Where this comes from
The official form is published by coronerscourt.vic.gov.au; this page is a completion guide. Last checked on 2026-10-23.
When to use it
Use this when a person, including a family member, has been summonsed to appear as a witness at an inquest and asks the Court to reimburse a loss or expense. A family member who attends without a summons cannot use it.
Used for these tasks
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
The tool indexes the claim headings and does not write the statutory declaration, the employer certification, or the witness declaration.
AI may
- Record that a claim is available only if the person was summonsed to an inquest, and diary the date evidence was given.
- List which of sections A to E the file already has receipts for, without entering the amounts.
AI must not
- Do not write the statutory declaration for a self-employed witness, the employer's certification, or the witness declaration.
- Do not enter a maximum allowance in place of the actual net loss.
- Do not claim petrol, parking, tax or GST. The form says those are not claimable on a private-vehicle or income claim.
- Do not prepare a witness statement. This form is the expense claim only.
The lawyer checks
- The form says authority is section 74A of the Coroners Act 2008, claims relate to inquests only, and mention hearings and directions hearings are not covered. The amount is determined in accordance with the court rules.
- An employed witness uses the employer certification. A self-employed witness uses the statutory declaration. A Court Registrar can witness that statutory declaration. Both boxes are not completed.
- The opened form prints the maximum hourly and daily rates, 18 cents a kilometre one way, the meal figures, and $150 a night for accommodation. Claim the actual net loss, not the maximum.
- The form footer prints 65 Kavanagh Street, Southbank VIC 3006, telephone 1300 309 519 and fax 1300 546 989.
- Health information stays in a firm-controlled tool. Check ASCR r 9, APP 11, and whether any disclosure would be a notifiable breach under the Privacy Act 1988 (Cth).
Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
