Where this comes from
The official form is published by coronerscourt.vic.gov.au; this page is a completion guide. Last checked on 2026-10-23.
When to use it
Use this when the Court asks a family member, or someone else who knew the person, to complete a statement of identification. It records the identification. It is not a witness statement about the death.
Used for these tasks
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
The tool indexes the identification fields and does not write the belief sentence or sign as the person or the witness.
AI may
- Index the court reference, the deceased's name and the relationship from the file.
- Record the place, date and time of the identification once the Court or the family has stated them.
AI must not
- Do not write the sentence that the information is believed true and correct.
- Do not sign as the person making the identification or as the witness.
- Do not turn this form into a witness statement about how the person died.
The lawyer checks
- The forms page says this statement is used to confirm identity. The senior-next-of-kin page says identification may be visual, or by fingerprints, dental records, DNA or other scientific methods. This form is the visual identification statement.
- The person inserts the belief sentence and signs. A witness then signs, with organisation, position or rank, contact number, date and time.
- The form prints telephone 1300 309 519, fax (03) 9682 1206 and email CAE@vifm.org.
- Health information stays in a firm-controlled tool. Check ASCR r 9, APP 11, and whether any disclosure would be a notifiable breach under the Privacy Act 1988 (Cth).
Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
