Where this comes from
The official form is published by coronerscourt.vic.gov.au; this page is a completion guide. Last checked on 2026-10-23.
When to use it
Use this when a family member asks the coroner to release the body for a funeral. The funeral-director release form is a different document and is completed by the funeral director. Lodge Form 25 with Coronial Admissions and Enquiries.
Used for these tasks
- Check the authority required before human remains are buried or cremated
- Map who is the senior next of kin and what each person may decide
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
The tool indexes Form 25 and does not write or sign the confirmation that the information is true and correct.
AI may
- Index the applicant, the deceased and the funeral director from the file.
- Diary 48 hours from the coroner's determination as the period the form's note gives for an appeal under section 85.
AI must not
- Do not write or sign the confirmation, and do not write the reasons for the application.
- Do not treat the funeral-director release form as Form 25.
- Do not name the court that hears a section 85 appeal. The opened extract does not complete that sentence.
The lawyer checks
- The form is headed Form 25, Rule 51, section 48 of the Coroners Act 2008. Lodge it with Coronial Admissions and Enquiries at cae@vifm.org.
- The note says a person who applied under section 48 may appeal under section 85 against an order to release the body or the terms of that release, and that the appeal must be made within 48 hours after the coroner's determination.
- The funeral page says the funeral director completes a separate application for release of a deceased person, and that if two or more people apply the coroner decides who the person is released to.
- The form says the application may be provided to any other applicant if the coroner decides that is appropriate.
- Health information stays in a firm-controlled tool. Check ASCR r 9, APP 11, and whether any disclosure would be a notifiable breach under the Privacy Act 1988 (Cth).
