Where this comes from
The official form is published by coronerscourt.vic.gov.au; this page is a completion guide. Last checked on 2026-10-23.
When to use it
Use this when a family member or their lawyer asks the coroner for access to, or release of, a thing or sample the investigation holds. The forms page says it does not apply to tissue samples or other material removed from the body.
Used for these tasks
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
The tool indexes Form 34 and does not write the reasons or the confirmation that the information is true and correct.
AI may
- Index the applicant, the legal representative and the deceased from the file.
- List the things or samples the family has already identified, without stating why release is sought.
AI must not
- Do not write or sign the confirmation.
- Do not write the reasons for the application.
- Do not use this form for tissue or other material removed from the body. The forms page says it does not apply to that material.
The lawyer checks
- The form cites rules 60(3) and 62(2) of the Coroners Court Rules 2019 and section 114 of the Coroners Act 2008.
- Mail is 65 Kavanagh Street, Southbank, VIC 3056, as printed on the form. Email is courtadmin@courts.vic.gov.au, or the team address if known. Phone 1300 309 519.
- If a legal representative is named, the form says the requested documents are released to that representative. Include the authority to act.
- The confirmation says the information is true and correct to the best of the applicant's knowledge. The applicant signs it.
- Health information stays in a firm-controlled tool. Check ASCR r 9, APP 11, and whether any disclosure would be a notifiable breach under the Privacy Act 1988 (Cth).
Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
