Where this comes from
The official form is published by coroners.nsw.gov.au; this page is a completion guide. Last checked on 2026-10-23.
When to use it
Use when a family, or a lawyer for the family, asks the Coroners Court of New South Wales for documents from a current or finished coronial file. The forms index lists this as the only form. Deaths before 1963 are directed elsewhere on the access page.
Used for these tasks
- Map who is the senior next of kin and what each person may decide
- Index the coroner's brief and list the records still to be obtained
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
The tool may list the documents already held and the fields still blank, and it writes nothing in the reasons, the relationship, or the signature.
AI may
- List which of the form fields are still blank from the facts already on the file
- Index documents already held against the checkboxes for a medical examination report, a police report, and findings or recommendations
- Note the fee lines on the current forms page beside the documents requested, for the lawyer to check
AI must not
- Must not decide who is the senior next of kin, or write the reasons a person who is not the senior next of kin gives for the request
- Must not sign the form or send it to the court
- Must not describe a post mortem report, a brief, or findings that are not on the file
- Must not prepare a request for an inquest, an objection to a post mortem, or an application for leave to appear. Those are not on this form
The lawyer checks
- The PDF in use is the current Form 24. The copy opened on 23 October 2026 is marked modified 1 February 2025. The forms and fees page was last updated 8 June 2026 and listed this as the only form
- The applicant's relationship to the person who died is taken from a document. A person who is not the senior next of kin states reasons, and the access page says other family members attach proof of the relationship
- The form is sent to the Local Court where the coroner who determined the cause and manner of death was attached. If that court is not known, the form says a death certificate from the Registry of Births, Deaths and Marriages should name it. If it is still not known, the form gives the NSW State Coroner's Court, 1A Main Avenue, Lidcombe NSW 2141
- The form prints "current colonial matters" in the fee exemption. The forms and fees page prints "current coronial matters". Quote the document being used
- Findings addresses differ on the two sources. The form prints www.coroners.justice.nsw.gov.au. The forms page prints www.coroners.nsw.gov.au/findings. Use the address on the page being followed
- Historical files from 1963 to 2000 need Form 24 plus the death certificate and proof of relationship, on the access page. Deaths before 1963 are directed to State Records. The access page says to allow up to 12 months for a historical file
- The fee regulation named on the forms page was not opened. Fees are taken from the current forms page. The access page dates two of those figures at 1 July 2024
- Health and other personal information stays in a firm-controlled tool. Check confidentiality under ASCR r 9 and APP 11, and whether any breach is notifiable under the Privacy Act 1988 (Cth).
