When to use it
Use when a visa or citizenship application needs the declaration of service the character page calls Form 1399. It is not Form 80 and it is not Form 1563.

- Do not draft, settle or sign an affidavit, statutory declaration or other sworn evidence.
- Do not lodge the form or tell the client it has been filed.
The lawyer checks
- The character page names Form 1399, Declaration of Service, including where a military certificate is requested.
- The PDF says documents such as military service or exemption papers, with translations, must be submitted as the questions indicate.
- If clarification is requested and not answered in the set time, the PDF says a decision may be made without further steps to get the information.
- The signature block was not read. The lawyer completes any declaration on the current PDF.
