Build a page-cited New South Wales treatment chronology for a claim
MaintainedLast reviewed
Key facts
- Human involvement
- YellowClose supervision
- How AI is used
- From records held in a firm-controlled tool, build a dated treatment chronology with the provider, the complaint, the finding and the page of each row, and list the gaps. Do not diagnose, and do not fill a gap.
- What the lawyer must review
- Check every page cite against the record. A missing page is a gap, not a finding
- In the Commission, lodge an indexed and paginated bundle, and do not re-lodge material already filed
- PIC3 paragraph 9 requires PIC13 compliance, and the index counts towards the 500-page limit
- Keep the records in a firm-controlled tool. A leak may be notifiable

The work product
Forum. New South Wales Personal Injury Commission, or a New South Wales court
Produce. A page-cited treatment chronology, with gaps listed, for the lawyer to check.
Have ready. Medical records held in the firm's system, with page numbers; The forum, if a bundle is being lodged; Any direction about documents already lodged.
Example of the limit. Illustrative example: Three attendances are listed. Two match the cited pages. The third page is a referral letter and does not record that attendance. The lawyer deletes the row and leaves the gap.
Stop and hand it back when
- A row has no page cite, or the cited page does not contain the entry.
- The bundle re-lodges material already filed, or the index is left out of the page count.

Who does what
| Role | What they do |
|---|---|
| Assistance | From records held in a firm-controlled tool, build a dated treatment chronology with the provider, the complaint, the finding and the page of each row, and list the gaps. Do not diagnose, and do not fill a gap. |
| Lawyer | Set the scope, supervise the work closely, verify every part relied on, resolve omissions and exceptions, and approve the exact output. |
Checklist
- Procedural Direction PIC3 requires lodged documents to be indexed, consecutively paginated and legible, without duplicates. Material already lodged is referred to in the index and is not lodged again.
- PIC3 says the index counts towards the rule 67B 500-page limit even where the index itself is not numbered. Documents in each category are in chronological order.
- PIC3 paragraph 9 requires compliance with PIC13 for a chronology. PIC13 paragraph 11(a) allows a chronology. Paragraph 12 says generative AI must not generate the content of a person's evidence.
- Practice Note SC Gen 23 paragraph 9B(a) allows a chronology. Paragraph 10 prohibits generating the content of an affidavit or the witness's evidence. The Local Court practice note commencing 12 January 2026 adopts that practice note.
