DMCA notice of claimed infringement
Checklist and cover fields for a notice of claimed infringement under 17 U.S.C. § 512(c)(3). It identifies the work, the location and the designated agent. It does not contain the good-faith statement or the statement under penalty of perjury.

When to use it
Use when a rights holder asks a service provider to remove material and the provider has a designated agent. Do not use it as a court pleading, a counter-notice, or a letter to the alleged infringer. The rights holder writes the two statutory statements before anything is sent.
Used for these tasks
- Draft a cease-and-desist letter for IP infringement
- Build an infringement claim chart from the asserted rights and the product
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
Rated red. One element of a valid notice is a statement under penalty of perjury. The tool may list locations and work titles the lawyer has already identified. It does not write the notice that will be sent.
AI may
- List the URLs and work titles the lawyer has already identified, in a working table
- Compare a lawyer's draft with elements (i) to (iv) of § 512(c)(3)(A) and mark what is missing
- Record the designated-agent name and address from the directory page the lawyer opened
AI must not
- Must not draft element (v) or element (vi), or any statement under penalty of perjury
- Must not assert that material is infringing, or that the sender is authorised to act
- Must not draft a counter-notice, and must not send the notice
The lawyer checks
- The notice goes to the provider's designated agent, checked on the current Copyright Office directory
- The rights holder, not a tool, writes the good-faith statement and the statement under penalty of perjury
- The work, the location and the sender's authority have been checked against the file before anything is sent
