PTO/AIA/14 application data sheet for a utility or design application
Link to application data sheet PTO/AIA/14. The fillable PDF is marked PTO/AIA/14 (03-26) and approved for use through 11/30/2027. The forms index still prints 01/2022. For a new utility or design filing in Patent Center, the USPTO recommends a Web-based ADS rather than the PDF.

Where this comes from
The official form is published by uspto.gov; this page is a completion guide. Last checked on 2026-10-22.
When to use it
Use when bibliographic data for a U.S. utility or design application is given on an application data sheet under 37 CFR 1.76. A nonpublication request can be made only at filing, and only on a properly signed ADS.
Used for these tasks
- Draft a patent specification from the inventor disclosure
- Check named inventors on an AI-assisted patent application
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
The tool may compare bibliographic fields on a lawyer-prepared ADS with names and dates already in the file, and it must not write an inventor's belief, a nonpublication request, or a signature.
AI may
- Compare applicant, inventor and application-number fields on a draft ADS with a correspondence schedule the lawyer has already approved
- Check that domestic benefit claims on a draft are listed from newest to oldest, which is the order the ADS page says the Web ADS is built to support
- Flag a nonpublication request that is not being made at filing
AI must not
- Must not state that a person is an inventor or the original inventor
- Must not request nonpublication, or decide that an ADS signature makes that request effective
- Must not sign the ADS, or apply an S-signature for the named signer
- Must not mark a corrected ADS unless the lawyer has identified each addition and deletion
The lawyer checks
- For a new utility or design application filed in Patent Center, the ADS page recommends a Web-based ADS rather than the PDF of PTO/AIA/14
- An ADS filed after the application is submitted, even if none was filed before, must identify additions with underlining and deletions with strike-through or brackets
- A nonpublication request can be made only when the application is filed, and the ADS must be properly signed for that request to be effective
- If a juristic entity is an applicant, a patent practitioner signs the ADS. If two or more joint inventors are the applicant, all of them sign, or one or more who hold a power of attorney, for example PTO/AIA/81, sign for all
- The named signer personally inserts the signature. The page allows an S-signature, a handwritten signature, or a graphic of either
- The forms index says that presenting an Office form is a certification under 37 CFR 11.18(b) that existing text and certification statements have not been altered except as the patent electronic filing system permits. See also 37 CFR 1.4(d)(3), which the index cites and which was not opened separately
