Advance Personal Plan (Northern Territory): completion guide
Link to the Northern Territory Advance Personal Plan form. The page gives no form number. From 2014 this plan replaced enduring powers of attorney, and no new enduring power of attorney can be created. Documents made before 2014 can still be used.

Where this comes from
The official form is published by pgt.nt.gov.au; this page is a completion guide. Last checked on 2026-10-22.
When to use it
Use when an adult is making a new Northern Territory Advance Personal Plan, including an advance care statement, advance consent decisions, or appointment of a decision maker. Do not create a new Northern Territory enduring power of attorney. The tool does not write the plan, the wishes or the witness confirmation.
Used for these tasks
- Draft an enduring power of attorney (financial) from the instructions
- Draft an enduring personal/health decision-making appointment
- Draft an advance care directive from the instructions
- Build the execution and witnessing plan for an enduring instrument before signing
- Review existing enduring powers, guardianship orders and advance care directives
- Build a decision-making map: who holds which power, from when, and over what
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
AI may check the pack against the current form and list missing documents. The lawyer writes the instrument, the powers, the wishes and any witnessing or capacity words, and checks every signing.
AI may
- Check the pack against the current official form and list missing parts named on the page
- Flag when an enduring power, a health direction, a statement of choices or an advance personal plan is being used for the wrong purpose
- List the witness categories and the registration path stated on the page that was opened
AI must not
- Must not draft the instrument, the powers, the person's wishes, a treatment refusal, or witnessing or capacity words
- Must not copy an example statement from a public page into the person's document
- Must not sign or lodge the instrument
The lawyer checks
- The current form is the one linked from the Public Guardian and Trustee page. No form number was stated there
- A new Northern Territory enduring power of attorney is not prepared. Pre-2014 documents are reviewed rather than reissued as a new enduring power
- The witness is in a category the page lists, and the witness confirmation is the witness's own
- The court's or tribunal's current index is checked before assuming a generative-AI practice direction applies. None was stated on the page opened for this guide.
Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
