Where this comes from
The official form is published by publicguardian.tas.gov.au; this page is a completion guide. Last checked on 2026-10-18.
When to use it
Use when a Tasmanian client wants to appoint an enduring guardian for personal and lifestyle decisions (health care, where they live, support received) if they lose decision-making ability.
Used for these tasks
- Draft an enduring personal/health decision-making appointment
- Build the execution and witnessing plan for an enduring instrument before signing
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
AI may help prepare the instrument from the client's confirmed instructions and plan execution and registration; the lawyer takes instructions from the client alone and checks capacity, every entry, execution and registration.
AI may
- Transfer confirmed instructions (guardians, alternatives, conditions and limits, wishes) into a checking sheet
- Check the completed instrument against the instructions and list every difference
- Build the registration step with TASCAT through Service Tasmania
AI must not
- Must not take instructions from anyone other than the appointor
- Must not assess capacity
- Must not choose the guardians, conditions or limits
The lawyer checks
- Decisions covered (full authority or limited) and any conditions match the instructions
- Joint guardians understand they must act jointly; any alternative guardian named
- The client's wishes recorded as they want them taken into account
- Instrument registered with TASCAT through Service Tasmania
