Evidence pack index for an NDIS registration audit
An index for the documents a provider gathers before an NDIS registration audit: the scope, the stage, the sites, and the participant notice. It does not invent the verification document list.

When to use it
Use when a provider is preparing for a verification audit, a certification audit, or the mid-term audit described on the NDIS Commission types-of-audits page. Do not use it for a condition audit or an out-of-cycle audit until those sections of the page have been read. They were not.
Used for these tasks
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
AI may fill the audit type, the stage, the site addresses and the document titles from the provider's file. The lawyer checks that each document is the one the current scope of audit asks for, and that participants have been told about the onsite audit.
AI may
- Enter the audit type and stage from the initial scope of audit
- List site addresses and document titles that are already on the file
- Record the date participants were told, and any opt-out, without copying a participant's health information into the index
AI must not
- Must not invent the verification document list. That guide was not opened
- Must not copy the worked example on the Commission page
- Must not describe a non-conformity as a failed audit
- Must not put a participant's name or health information into an unapproved tool
The lawyer checks
- The audit type matches the initial scope of audit, and the registration-groups table has been read for that group
- Participants have been told they are enrolled unless they opt out, before a Stage 2 audit
- Stage 2 is diarised within 3 months after Stage 1 is complete, where the audit is a certification audit
- Quotes are from approved quality auditors. The Commission does not set the audit price
Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
