Where this comes from

The official form is published by nt.gov.au; this page is a completion guide. Last checked on 2026-10-22.

When to use it

Use on a family file when both parents, or guardians, apply to change the name of a child over 12 months who was born in the Northern Territory or overseas. A child born interstate is applied for in that state or territory. If both parents are on the birth certificate, both must apply. The tool does not write the reason or the lost-certificate statutory declaration.

Used for these tasks

Each task page says how far AI may help with the task and what the lawyer must review.

How AI may be used

AI may check that the both-parents file matches the child's place of birth and list the page's documents. The lawyer writes the reason and checks the application before it is lodged.

AI may

  • Choose the NT-born file or the overseas-born file, and flag an interstate birth as out of this page
  • List the signature block: both parents or guardians, any other interested person, and the child if over 14
  • Flag a missing previous birth certificate, which the page sends to the statutory declaration

AI must not

  • Must not draft the reason for the change. The page rejects 'we want to' and 'personal'
  • Must not draft, complete or attach words to the lost-certificate statutory declaration
  • Must not draft a parenting plan or parenting orders
  • Must not invent a fee or a form number
  • Must not sign, witness or lodge the application

The lawyer checks

  • Both parents apply where both are named on the birth certificate. Guardians use the guardians' test on the page
  • A child not born in the Northern Territory needs 12 consecutive months of residence, proved as the page describes, unless the child was born interstate, in which case the application is made in that place
  • Signatures are in front of a witness over 18. The application itself is not called a statutory declaration. If it is one, the tool writes nothing in it
  • The fee page was not opened
  • The court's or tribunal's current index is checked before assuming a generative-AI practice direction applies. None was stated on the page opened for this guide.
