Where this comes from
The official form is published by vetboard.vic.gov.au; this page is a completion guide. Last checked on 2026-10-22.
When to use it
Use when a client in Victoria wants the Board to consider the professional conduct or health of a named veterinary practitioner. The client writes to the practitioner first unless the lawyer decides the page allows otherwise.
Used for these tasks
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
AI may build a dated table from the clinical records; the lawyer and the client write the complaint, including the declaration that the information is true.
AI may
- Build a dated table of consultations, procedures and fees from the records the lawyer marks
- List the practitioner full name from the Board register if the lawyer has opened it
AI must not
- Must not write the client account of what happened
- Must not complete or sign the declaration that the information is true and correct
- Must not say whether the Board can investigate, or lodge the form
The lawyer checks
- The concern was raised with the practitioner in writing, and two weeks have passed or the response is unsatisfactory, unless the lawyer decides otherwise from the page
- One form for each practitioner, using the full name
- The complaint is not only about prices, a practice, or a person who is not a practitioner, unless the page allows it
- The client makes the declaration. The tool does not
Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
