Where this comes from
The official form is published by publications.qld.gov.au; this page is a completion guide. Last checked on 2026-10-22.
When to use it
Use when a person who has been given an information notice wants an internal review of an animal welfare direction, a seizure direction or a forfeiture notice. Apply within 14 days. The application does not stay the decision.
How AI may be used
AI may list the notice, its date and the orders it contains; the lawyer decides whether to apply, writes the grounds, and the client lodges Form 1170.
AI may
- Record the decision named in the notice, the date the information notice was given, and the fourteenth day
- List the orders in the direction in the words of the notice
- Flag an application that would be outside 14 days of that date
AI must not
- Must not decide that the direction should be changed or set aside
- Must not write the grounds or lodge Form 1170
- Must not say the direction is paused because a review has been lodged
The lawyer checks
- The applicant is an interested person as the Act uses that term. The form says the Act defines it
- The application is made within 14 days of the date the applicant is given the information notice
- The original decision continues unless a Magistrates Court stays it under section 196
- The form is sent to the address or the email printed on the current Form 1170
