Where this comes from
The official form is published by uscourts.gov; this page is a completion guide. Last checked on 2026-10-22.
When to use it
Use when counsel needs the current district-court form by which the clerk certifies admission and good standing. The opened PDF is the certificate the clerk signs. It is not a party affidavit, and no live task asks for that certificate.
How AI may be used
The tool may record that the opened form is the clerk's certificate and list the blanks on it, and it must not draft the certificate or say that a lawyer is in good standing.
AI may
- Remind the lawyer that the opened PDF is signed by the clerk, not by the applicant, and that the extract contains no perjury declaration
- Remind the lawyer not to merge a court-of-appeals certificate into this district-court form. No appeals variant was opened
AI must not
- Must not draft the clerk's certificate or fill the blanks as if the certificate had been issued
- Must not state that a lawyer is admitted or in good standing
- Must not add a perjury declaration. The opened extract does not contain one
- Must not describe a request affidavit. The opened PDF is the certificate itself
The lawyer checks
- The form page gives the form number AO 136 and an effective date of 1 October 2013. The title is Certificate of Good Standing (District Court)
- The opened PDF is headed AO 136 (Rev. 10/13) Certificate of Good Standing. It is the clerk's certificate: the clerk certifies that the named lawyer was duly admitted and is in good standing
- The extract is short. It does not contain a party request, a supporting affidavit, or a perjury declaration. Do not invent a request form
- No court-of-appeals good-standing form was opened. Do not merge one into AO 136
