- How AI is used
- Index the invoices, contracts and payment records that support the amount, and note which documents are writings on which the claim is based. Do not fill in Official Form 410.
- What the lawyer must review
- The proof must substantially conform to Official Form 410 and be signed by the creditor or the creditor’s agent, except as Rules 3004 and 3005 provide (Federal Rule of Bankruptcy Procedure 3001(a) and (b)). A claim based on a writing needs the writing filed with the proof, subject to Rule 3001(c). The courts’ form page identifies the current Official Form 410 as effective 1 December 2024. The lawyer completes and files the form. Any declaration on the official form is the signer’s, and the tool writes none of it.
