- The lawyer decides who is sued and where an initial notice must go.

The work product

Produce. A table of clinicians, with the section 9A name and place marked.
Have ready. Treatment records the lawyer selects; Whether the proposed defendant is the State.
Example of the limit. Illustrative example: The note names a doctor and a public hospital. The table records both and marks section 9A(7) as the address question. It does not choose the defendant.
Stop and hand it back when
- The tool is asked to choose the defendant.

Checklist
- One row per person the record names, with role, place and page.
- Mark whether the doctor who provided the medical services is known, and the place, because section 9A(3)(f) and (h) require those for the initial notice.
- If the proposed defendant is the State, record whether the place is under a Hospital and Health Service. The notice is then given to that health service chief executive, or otherwise to the chief executive officer of the department or entity.
- Part 1 against the State is a different address. Section 9(8) requires the Crown solicitor and the department or entity. Do not use that address for the initial notice.
