| Assistance | From the departmental records the lawyer has selected and cleared for processing — case notes, notification and risk-report summaries, prior assessments and file correspondence — extract each dated entry into a chronology table giving the date, the document, the page, who recorded it, and whether the entry records what the worker saw or what the worker was told. |
| Lawyer | Set the scope, supervise the work closely, verify every part relied on, resolve omissions and exceptions, and approve the exact output. |
Checklist
- Confirm the record set is the complete production or subpoenaed holding for the period in issue, and identify what is missing or withheld before the chronology is relied on.
- Spot-check the date and author on a sample of entries against the source page, and check every entry the department relies on in its grounds.
- Check that each entry is marked observation or report, and correct any entry where an allegation, a caller's account or a diagnosis has been recorded as fact.
- Apply the statutory restriction on disclosing the identity of a person who made a report, and mask placement addresses and carer identities, before the chronology leaves the file.
