NSW reportable conduct: working sheet for the 7-day notification
A working sheet for the head of a NSW relevant entity preparing the Office of the Children's Guardian's 7-day notification: the form fields, the written delegation, and what must not be guessed. The notification itself is written by the head of entity or their delegate.

When to use it
Use when a NSW school or other relevant entity has a reportable allegation and must notify the Office of the Children's Guardian. Confirm on the current form when the period starts. This sheet does not decide whether the allegation is reportable conduct.
Used for these tasks
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
AI may list which form fields are still blank and build a chronology from documents the lawyer has identified. The head of the relevant entity, or a delegate, writes every sentence of the notification that is submitted.
AI may
- List the form fields in the OCG guidance and mark which ones the file does not yet answer
- Build a chronology of dates from emails and incident notes the lawyer has marked as source documents
- Record whether a written Part 4 delegation is already on the file
AI must not
- Must not draft the description of the allegation that will be submitted
- Must not decide that conduct is or is not reportable, or that it is or is not connected to work
- Must not include the names of children in a tool that is not approved for that information
- Must not submit the form
The lawyer checks
- The entity is a relevant entity and the worker is covered. That decision is not made by this sheet
- The person notifying is the head of the relevant entity, or a delegate whose Part 4 functions were delegated in writing and notified to reportableconduct@ocg.nsw.gov.au
- Work-connection is marked yes, no or unknown on the facts known today, and unknown is not left blank
- The form is submitted by a person, within the period the current form and the Act require, which must be read rather than assumed from the form's name
Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
