Build a medical chronology and mark entries with no source page
Key facts
- Human involvement
- YellowClose supervision
- How AI is used
- One row per record entry: date on the page, provider, and the finding in the words of the page. Mark a row the tool cannot tie to a page. Do not add a diagnosis or a cause.
- What the lawyer must review
- 45 C.F.R. § 164.502(a) limits use and disclosure of protected health information by a covered entity or business associate. Not every firm is one. Model Rule 1.6 still keeps the records out of a public tool. Formal Opinion 512 requires review before the chronology is relied on, because a tool can add a sentence that is not on the page. You accept each row.
