- How AI is used
- List the topics the lawyer says the statement must cover. Write no statement, no function report and no declaration.
- What the lawyer must review
- A statement the person will sign is that person's account. Where federal law allows proof by a sworn statement, 28 U.S.C. § 1746 allows an unsworn declaration the person subscribes as true under penalty of perjury, in the form the section sets out. The words remain the person's. You do not file generated text as the claimant's account, whether the forum is the Social Security Administration or an employer accommodation file. Medical detail is protected by Model Rule 1.6. Formal Opinion 512 requires independent verification and does not let a tool stand in for the person's recollection.
