- Official Form 105 is the involuntary petition against an individual. Its face says to use Official Form 205 against a non-individual. Both forms require the chapter, the debtor's identity and address, venue, the § 303(b) and § 303(a) allegations, at least one of the two grounds, and each petitioner's claim. Form 105's request for relief includes a declaration that the information is true and correct and refers to 18 U.S.C. §§ 152 and 3571 and to 11 U.S.C. § 303(i). Form 205 includes a declaration under penalty of perjury and warns of a false statement under 18 U.S.C. §§ 152, 1341, 1519, and 3571. You sign. Bankruptcy Rule 9011(b) is the certificate made by presenting the petition. The 21-day chance to withdraw a paper before a sanctions motion is filed does not apply to filing a petition.
