- List the dates, placements, services and orders in the file, each with the document and the page. Write no sentence of the parent's account and no outline of what the parent will say.
- What the lawyer must review
- The account that is filed is the parent's evidence, and you write every sentence of it. The index is a map of the file. Section 675(1) defines the case plan as a written document that includes a plan for safe and proper care. Section 671(a)(15)(A) makes the child's health and safety the paramount concern in determining reasonable efforts. The California 2026 guidance says candor to the tribunal cannot be delegated, and Opinion 512 keeps the judgment with you.

Checklist
- Check the current court or regulator requirements, prescribed form and attachments for the intended document and filing.
- Confirm instructions and personal circumstances through appropriate direct engagement. Have the responsible lawyer assess safety, capacity or vulnerability concerns and escalation.
- Confirm the output against the client’s instructions and the complete relevant source material, including omissions or contradictions.
- Resolve exceptions and record the responsible lawyer’s approval of the exact output and intended use.
