Where this comes from
The official form is published by ombudsman.gov.au; this page is a completion guide. Last checked on 2026-10-22.
When to use it
Use when an agency must tell the Commonwealth Ombudsman of an allocation, a reallocation, a decision not to allocate, a decision not to investigate, an extension of time, a finalised investigation, or a stop action direction.
Used for these tasks
- Prepare the allocation and investigation worksheet for a public-sector disclosure
- Prepare the written reasons for not allocating or not investigating a public-sector disclosure
- Choose the Ombudsman notification for an allocation or investigation decision
Each task page says how far AI may help with the task and what the lawyer must review.
How AI may be used
AI may match the decision the lawyer has marked to the form title on the Ombudsman tools page and list the fields still empty. The lawyer completes and sends the form.
AI may
- Match one decision to one form title from the tools page
- List attachments the lawyer has said will go with the form
- Check the email address against the address printed on the tools page
AI must not
- Must not complete the form with the discloser's name or contact details
- Must not send the form
- Must not treat the form as the notice to the discloser under s 44 or s 50
The lawyer checks
- The current file is downloaded from the tools page for this notification
- The decision matches the form title, including Form 2 for a decision not to investigate and Form 4 for a decision not to allocate
- The form is emailed to pid@ombudsman.gov.au, the address on the tools page
- The section text is the 4 June 2026 compilation, not the earlier compilation linked from the tools page
Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
