When to use it

Complete at the first appointment in any matter where the client has experienced, or is at risk of, family or domestic violence, and before the firm sends any letter, email, text or bill. Review it whenever the client moves, changes phone or separates further. It sits with the safety screening checklist in this library.

How AI may be used

The form is a fixed firm template completed with the client; AI may set up the matter-system fields and reminders from it but must not hold the safe contact details or decide what is safe.

AI may

  • Convert the completed form into matter-system flags and contact rules, for a person to check against the signed form
  • Draft a reminder to the file owner when the review date arrives
  • Check outgoing correspondence lists against the recorded safe channels and flag any mismatch for a person to resolve

AI must not

  • Must not hold or process the client’s address, safe phone number, safe email or code word in any AI tool
  • Must not send, schedule or auto-generate any message, invoice or survey to the client
  • Must not decide that a channel is safe or override the client’s instructions
  • Must not be connected to a shared calendar, email or messaging account that the other party could see

The lawyer checks

  • The form was completed with the client alone, without the other party or their associates present or listening
  • Every automated message the matter system can send (texts, reminders, invoices, surveys, e-signature requests) has been switched off or pointed at the safe channel
  • The client’s address is withheld from court documents and correspondence to the other side unless the client consents
  • The review date is diarised and the form is updated when the client’s circumstances change
