Where this comes from
The official form is published by childrenscourt.nsw.gov.au; this page is a completion guide. Last checked on 2026-10-17.
When to use it
Use when acting for a parent, carer, child or other party in NSW care proceedings and you need to file or respond to an application. Do not use for other states, which have different Acts, courts and forms.
How AI may be used
Lawyer-led. Forms must be the Court's current versions, completed by the lawyer from instructions. Identifying information about the child, carers and placements must not go into AI tools that are not approved for it, and the Court applies SC Gen 23.
AI may
- Produce a filing checklist for a given application type from this guide
- Check a completed form for internal consistency of dates and order references in a firm-approved closed tool, with identifying details redacted
AI must not
- Must not complete affidavits or the narrative parts of any form
- Must not receive placement addresses, carer identities or reporter information
- Must not use a downloaded or cached copy of a form instead of the current version
The lawyer checks
- Current form version and the Court's completion guide
- Notice requirements (for example s 90(1A) and (3A)) and service
- No reporter identity or placement details disclosed in the documents
Method: how the Green, Yellow and Red levels are decided. This is guidance for planning, not legal advice.
