Build a treatment chronology for a negligence review
Key facts
- Human involvement
- YellowClose supervision
- How AI is used
- In date order, list the encounter, the provider, and the fact the record states, with the page. Do not diagnose and do not say a duty was breached.
- What the lawyer must review
- You decide whether any duty was breached. 45 CFR § 164.502 limits use and disclosure of the records. Section 164.524 requires a covered entity to act on an access request no later than 30 days after receipt, with one extension of no more than 30 days if it gives a written statement of the reasons and the completion date. You read the exception in § 164.524(b)(2) before using a different period. Formal Opinion 512 applies before the records go into a tool. This chronology does not also build a liability table.
